You push your target into an alien dimension, where they are subject to the cosmic horrors and mind-rending sensory overload. The creature vanishes from reality for the duration and when they return they immediately mindbreak. A successful save leaves them out of reality for only 1 round, and with 1d6 points of madness.
If a creature would perishes as a result of the mindbreak they instead do not return. What returns is a copy of their body which is left at 1 hp. It behaves as the original would, but after 1 hour its body destabilizes and it erupts into a pile of tendrils and otherworldly monsters as if summon monster with a caster level equal to your manifester level, and a spell level of 8th. Defeating these conjured creatures has the tendril pile spit out the insane original victim (as per mindbreak if it would reduce you to 0 hp) at 0 hp. Killing the doppleganger early destabilizes the body immediately, does not summon monsters, and returns the original insane, at -1 hp, and dying.
