Agreed with Fox (though I know this is old). Multi-turn wind-up mechanics are better in theory in D&D, and the one this thing uses is a pain to track, has extremely limited applicability and the only ways I can think of to make it useful encourage tedious and corny gameplay.

The special attack action which is supposed to make the Ball and Chain interesting doesn't seem good or odd enough, and conditionally increased critical multipliers seem like an annoying mechanic.
