This spell transforms you a new form and the new form must be any of the following types : [[aberration]], [[animal]], [[dragon]], [[fey]], [[giant]], [[humanoid]], [[magical beast]], [[monstrous humanoid]], or [[plant]]. You can’t choose to transform you into a swarm, incorporeal, or gazeous form. The new form must keep the same size category and can’t have more Hit Dice than your HD.

The transformation lasts for the spell duration, or until you drop your hit points to 0 or die. Your game statistics, including Wisdom, and Charisma ability scores, are replaced by the statistics of the chosen new form, but you retains your own intelligence. You also retains your alignment and personality.

You assume the hit points of your new form. When you revert to your normal form, you returns to the number of hit points you had before you transformed. If you revert as a result of dropping to 0 hit points, any excess damage carries over to your normal form. As long as the excess damage doesn't reduce your normal form to 0 hit points, you're not unconscious. You are limited in the actions you can perform by the nature of your new form.

Your gear and equipment meld into the new form. You can't activate, use, wield, or otherwise benefit from any of your equipment.

By casting this spell in a higher level spell slot (up to level 9), you can access stronger effects (multiple choices) :
